At Forever Cosmetics , your health and safety is our number one priority. We understand the severity of COVID-19, the importance of social distancing and enforcement of health department guidelines.
For your safety we have implemented additional infection prevention and control measures to our current workplace protocols in order to provide you with a safe space.

please note the following:

• All visits are by appointment only. 
• Please arrive at your appointment alone. We are limiting the number of clients in our studio in order to maintain appropriate social distancing. We are limiting the number of clients in our studio in order to maintain appropriate social distancing.
• When arriving at your appointment you will be asked to call the studio prior to arriving so to prevent you and the previous client from being in the studio at the same time. This also allows us to properly sanitize in between clients. We sanitize our facility thoroughly before and after every client, this includes disinfecting all surfaces, equipment, door knobs and seating areas.
• Temperature Check upon arrival. Please note: as per CDC standards, a temperature over 100 degrees Fahrenheit is a fever, if this happens to be the event you will be required to reschedule your appointment.
• All clients will be required to wear a mask. Due to the nature of the service and proximity between artist/client, it is important clients follow this protocol.  Please be advised, if this is not followed we will need to reschedule your appointment.
• All clients are required to complete a Covid- 19 questionnaire and informed consent form prior to their initial appointment and at their follow up.
• All Lip Procedures Must provide a negative covid test no earlier than 3 days prior to procedure. No exceptions. 
• If you are scheduling your first Covid-19 Vaccine, you must plan 1 week prior to your permanent makeup procedure. 
• No Permanent Makeup can be Performed between your First and second dose. 
• You must wait 10 days following your second vaccine dose before having a permanent makeup procedure.
